A referral from your primary care provider, chiropractor, spine surgeon, or other specialist is required. The referral should include the most recent office note, relevant imaging reports, and the reason for the referral. Referrals can be faxed to (330) 493-2569. A referral form is attached below.

Please bring your insurance cards, photo ID, and copay to your appointment. If you have any recent imaging reports such as X-rays, CT scans, or MRIs, please bring those as well. While your medications are typically listed in the referral, you may also bring an updated medication list.
